If you are interested in becoming a provider of Home and Community Based Medicaid (Individual Options, Level 1, or SELF Waiver) Services to individuals with developmental disabilities, you must be certified through the Ohio Department of Developmental Disabilities (DODD).

Here’s a brief overview of what to expect and some quick links to get started.

Independent Provider Certification

When you have received a user name and password use the Provider Services Management to start a provider application, navigate to a pending application, or view your provider home screen.

Agency Provider Certification

Once You’ve Been Certified by the DODD

Once you have received both letters of approval (an Initial Letter and a Final Letter of Approval), copies must be submitted to the Stark DD Board. Providers can also mail them to or drop them off with the receptionist at Whipple Dale Center located at 2950 Whipple Ave NW, Canton, 44708. Please send them to the attention of Loralynn Wharton.
